ONGOING CARE COORDINATION

  • Act as a Liaison for families both local and out of area

  • Medical appointment scheduling & coordination

  • Escort client to/from doctor's appointments & inform family/responsible party of results

  • Communicate with medical providers such as doctors and discharge planners

  • Consultation with third party providers & other professionals

  • Facilitating end of life care discussions (Advance Directive/POLST)

  • Grief & loss/bereavement counseling
