Care Management is a holistic, client-centered approach to caring for older adults or others living with a mental or physical disability. Working with families, Care Managers help provide answers and insight at a time of uncertainty.
With the guidance of a professional Care Manager (also known as an Aging Life Care professional), families are given the information and the tools they need to make choices that ensure quality care and an optimal life for those they love. This approach provides a sense of wholeness for the aging adult characterized by maximum independence, a sense of purpose, and confidence in their aging life plan. It also reduces worry, stress, and time off of work for family caregivers.
